Scope | Kuwait Oil Company is looking through this IFP (Invitation for Proposal) for internationally recognized software on incident investigation process with the aim to support the investigation of every incident that could lead to fatalities, injuries, equipment damage, environmental consequences or operating issues and to share the lessons learned. The software shall have minimum capability, but not limited to, as described hereunder: - It shall provide, but not limited to, a logic flow of sequence to input information, related to: date, time, location wise, specific location, activity involved, incident owner, type of injury, damages or spilled / released substance, quantities of spilled / released substance, type of equipment damaged, team, group, contractor, subcontractor, investigation team, interviewed personnel, sequence of events (before, during & after the incident), facts & evidences, conditions and actions involved, failure mode charts, evaluation of the emergency response. - It shall not allow moving to the next chapters unless the full information is input in the previous one. - It shall have the capability to be kept open as much time as needed to input and upload information keeping it as draft versions until final report is issued. - It shall provide a comprehensive process & tool to investigate incidents with systematic sequence of identifying immediate cause, critical factors, and root cause and behaviors (ABC Chart) which led to its occurrence. - It shall provide a chart for conclusions and recommendations linked with immediate causes, critical factors and root causes. - It shall allow to get back and editing previous chapters as needed. - It shall have flexibility to be linked with the Company’s online HSE reporting database (HSE Live) or e-Beams (Maximo) for retrieving the information required to support investigation process. - It shall provide a set of questionnaires based on type and category of incident to assist investigation interview of onsite witness and record. - It shall have inbuilt capability to suggest corrective measures and recommendations clearly categorizing as low, medium and high priority action item with implementation timeline to prevent the incident. - It shall take into account the category (such as minor, moderate & major) It has the flexibility to categorize incident potentials as (negligible, low, serious or catastrophic) as conclusions of the comprehensive investigation report. - It shall have the flexibility to attach necessary diagram, sketches, photographs and other relevant documents as a part of investigation report. - It shall have inbuilt capability to tailor made notification workflows for reports and actions derived from the investigations via e-mail depending on incident categories of potential or severity as per Company’s classifications. |
